Call 803-635-6099 during office hours to schedule an appointment. If you phone first thing in the morning, there are a few same day appointments that may be available. If you are requesting an appointment for a problem that is not urgent we may ask that you make an appointment that is convenient for you several days out.
If you are unable to keep your appointment, please notify us 24 hours in advance. Your time and your provider’s time are important to us. This way we may plan for another patient to take that appointment, and you may reschedule for a time that is convenient for you. Making that call will help the John A. Martin Primary Health Care Center run more smoothly for everyone.
If you are a prenatal patient, we try to schedule most of our prenatal patient visits on Tuesdays. However, we understand that it may be necessary for a prenatal patient to schedule an appointment on another day of the week.
What to Bring
- Photo ID or driver’s license
- Current insurance cards
- Social Security number
- Co-pay, co-insurance, and/or deductible payment
- Medicine bottles for all medicines (prescriptions, over-the-counter or supplements) you are taking
- Concerns/questions for your provider
- Names and phone numbers of any other doctors you are seeing
- Results of tests that were done at any other medical offices or Hospitals
- Updated Patient Forms (if applicable)

Whenever possible, prescription refill requests should be handled during your visit with your provider. If a prescription refill is necessary outside a normal office visit, you may call the office at 803-635-6099 and leave a message. Please include your name, date of birth, the medication name, and pharmacy name and number. Please allow 48 hours or 2 business days for processing. You may then contact your pharmacy directly tomake sure your refill has been completed.
No prescription refill requests will be completed after normal office hours or on weekends. We encourage you to request refills a few days in advance of when you will need them and to anticipate your need for refills before weekends or holidays. If the provider should need to see you before refilling your prescription, our staff will call you.
